California Dream Act Forms

Many of the documents that the Financial Aid & Scholarships office requests contain highly sensitive information like your name, address, Social Security numbers, how much money you earn, and where you bank. If that data falls into the wrong hands, you could end up being a victim of identity theft. It is important to protect your information by sending any sensitive information on documents in the most secure and reliable way possible.

Please note, all subsequent verification documentation submitted to the Financial Aid & Scholarships office must be signed by the student, and if parental data is included, a parent’s signature is required. Electronic signatures are not accepted at this time. Documentation submitted without required signatures will not be processed.

Methods of Submitting Forms

1. Drop off in-person at the Enrollment Center (Sage Hall, Room 1020)
2. Send by mail (Attn: Financial Aid & Scholarships, CSU Channel Islands, One University Drive, Camarillo, CA 93012)
3. Scan, encrypt and email to financial.aid@csuci.edu (please read email encryption instructions)

Verification Forms

  Your California Dream Act Application was selected for verification. As a dependent student in the Standard verification group, please submit this form to the Financial Aid & Scholarships office.
• Independent Dream Act Verification Worksheet ( PDF , 277KB)
  Your California Dream Act Application was selected for verification. As an independent student in the Standard verification group, please submit this form to the Financial Aid & Scholarships office.
  
• Dream Act Selective Service Verification ( PDF , 252KB)
  Submit this form if requested by the Financial Aid & Scholarships office with the appropriate documentation to clear your Selective Service Requirement.
